We are currently seeking a compassionate and skilled Care Manager Assistant to join our team and support our geriatric care manager in delivering exceptional care.

Position Overview:

The Care Manager Assistant will work under the direction of a geriatric care manager to assist with daily tasks, including wellness checks, transportation, social outings, light housekeeping, and personal care. This role is ideal for someone with a background in healthcare, social work, or direct care who is passionate about supporting older adults and helping them maintain their independence.

Key Responsibilities:

  • Assist with wellness checks, including monitoring clients’ physical and emotional well-being.
  • Provide transportation for clients to medical appointments, social events, and errands.
  • Coordinate and participate in social outings and activities.
  • Perform light housekeeping duties to maintain a safe and comfortable living environment for       clients.
  • Assist with personal care, including grooming, dressing, and other daily tasks.
  • Document care provided and report any changes in client condition to the care manager.
  • Work collaboratively with the geriatric care manager to ensure clients’ needs are met.
  • Maintain confidentiality and adhere to privacy regulations.

Qualifications:

  • Preferably a background as a Medical Assistant (MA), Licensed Practical Nurse (LPN), Social         Worker (SW), or similar healthcare role.
  • Will consider Certified Nursing Assistants (CNA) with extensive experience.
  • Previous experience working with older adults or in a caregiving role preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate, patient, and attentive to the needs of others.
  • Valid driver’s license and reliable transportation required.
  • Must be able to pass a background check and drug screening.
